I think the meaning of this song has to do with a relationship. Demi is the one who needs help mending/healing her heart. And she is stands up to the person who broke it in the first place, asking for help. But they keep saying they can fix her heart, and like the song says, "...you never really can fix a heart." And in the process, she tries to fix their heart.
To me this meaning is when you were in a relationship, that was NOT going Ok. And you think about It. And end up using self-harm to make the pain go away. But instead you end up with stitches and a broken heart. Reminder to those who are out there struggling with depression/self-harm, YOU ARE LOVED!!! i know it may not seem like it, but it's true!!!
